A Case of Neuroleptic Malignant Syndrome Developed in a Bipolar Patient

Abstract

|
A case of neuroleptic malignant syndrome developed during the treatment with the neuroleptcs and lithium in a male patient with the manic episode of bipolar disorder was reported with themanic episode of bipolar disorder was reported with the
clinical
features and laboratory results. Articles related to the overview and the mechanism of the neuroleptic malignant syndrome were also reviewed.
Being preceded by prodromal symptoms beginning on the 40th admission day, sudden rising of temperature, worsening of extrapyranmidal symptoms such as muscular rigidity and tremor, clouding of consciousness, autonomic instability like diaphoresis,
tachycardia, urination difficulty, constipation as well as leukocytosis, increase of serum creatine phokinase and aldolase, and increase of serum myoglobin were noticed on 45th admission day, which were consistent with the criteria of full-blown
neuroleptic malignant syndrome suggested by Levenson(1985), Pope et al(1989).
Within 5 days, every vital sign including body temperature was normalized with the supportive measures and benzodiazepines applied just after the occurrence of fever with the discontinuance of every psychotropic drug.
